Heart Valve Disease Pathway Improvement Funding 2024/25


NHS England's Cardiac Transformation Programme is inviting Expressions of Interest (EoI) to fund pathway improvement projects which will have a positive impact on reducing the time for referral to treatment for patients with severe, symptomatic left sided valvular disease in England during 2024/25.


The funding should be spent by the end of 2024/25 and projects should aim to be completed within 12 months of receipt of the funding. Examples of projects are below. However, the national team are interested in any local innovation that could support the aim of the funding above.

Improve referral processes, e.g. automated triage processes, development of MDT support for local community. If a local valve clinic is not in operation, collaborative working across the network should be demonstrated.

Improve local diagnostic pathways, including community diagnostic centres
